I look forward to spending time with you on your chosen day.

To help me suss out things like dietary requirements (if your day includes catering) accessibility and learning needs.

As well as hearing from you what you feel would be helpful as themes for your day. This confidential form is something I ask all team members to contribute too, to help me co-create your day.

Depending on your learning style and needs allow anything from 5-25 minutes to complete the form. I will have let your team know when I need this information back by.
